Although the Leprosarium had doctor and nursing staff, living on the island was miserable. A large slum, without organization. A small monthly allowance granted by the state, was unable to meet their basic needs. Those who were better than the others are trying to secure their food either cultivating vegetables, or  fishing. There weren't  few cases of patients escaping from the island to nearby villages in search of food. The "fugitives" who were perceived, but also other "offenders", glued to a punitive prison was on to a rock (from a point on the prison was abolished). Many died helpless and in terrible pain, deformed, blind, or mutilated by the disease. The prefect of Lassithi in a letter on August 6, 1925, to the Eleftherios Venizelos is quite scathing about the appropriateness of Spinalonga as a Hospital: "And as a convict prison and as a grave is still insufficient. During my visit i had the worst impressions and perhaps only the imagination a Dante is enough to describe. Pro two hundred wretched human beings, all ages, social status and gender and physical strain, are there in full despair, without sense of moral or written laws.... " And the director of the Pasteur Institute "in Tunis, Nicole Charles, describes in his report at late 1930's, the situation in Spinalonga gloomy, asking the Greek government to close down the hospital, stating among others:" As only entertainment and employment, lack of work looking at the beach, play some games and some musical instruments. Most of the time, darn their luck, drink, getting drunk, quarrel and love. When we visited the island were not even systemic treatment of leprosy. In this tragic place while walking, followed, surrounded from all over the unaffected population, raise two questions in our minds: What if inadvertently exists between them and a healthy one?How is his life and what was the desperation he had if he knew... And among the lighter-infected (because the first manifestations of leprosy are light) how many do not consider themselves unjustly locked up in the island, condemned to spend all of their lifes there ... Many fell and drowned in the sea to escape from that prison, but some managed to go away swimming ....
